VOLUNTEER STATUS REPORTS
These Reports will provide you with your current Special Olympics Kentucky volunteer approval status.  Statuses included in the following two reports are: 

  • Volunteer Application Expiration Date (required every 3 years).
  • Volunteer Protective Behaviors Expiration Date (required every 3 years).
  • Volunteer Orientation Completion Status (required once with continued program involvement).
  • CDC Heads Up Concussion Training Expiration Date (required every 3 years)
  • U18 and Over 18 years of age Unified Partner Form on file.

Note:  These two reports contain the same information.  They are simply sorted differently in an effort to make it more convenient to locate your status.  Additionally within the report you will possibly see a (code) within an individual’s name.  These codes are translated to the following:

(P) = Pending – all items have been received and awaiting background results.
(MP)  = Missing photo or State issued ID
(MUP) = Missing Unified Partner Form.  This could mean nothing on file or volunteer has recently turned 18 requiring it to be completed as an adult.
(I) = Incomplete form.  This may include missing references, social security number, or did not sign application are examples.  Please resubmit ENTIRE form
(*)  = Submitted on sport season registration for an upcoming games but we have not received an application.
(U14)  = these individuals are under the age of 14
(U18)  = these individuals are under the age of 18

DO YOU NEED TO RENEW YOUR CLASS “A” VOLUNTEER STATUS?
To renew your Class “A” Volunteer status complete the following 3-4 steps. 

  • Complete and submit the Class “A” Volunteer Application.  Application can be downloaded from the resource page.
    • Please read carefully and completely the first page of the Application.
    • By completing this application you are authorizing Special Olympics Kentucky to complete a National Background check for those 18 years of age and older and the required reference check for those under 18 years of age. 
    • Completing this application is required every THREE years. 
  • Complete the online Special Olympics International Protective Behaviorscourse.
    • Not required of volunteers under the age of 18.
    • Required every THREE years for those over the age of 18.
  • Complete the online CDC’s “Heads Up” Concussion Training for Special Olympics Kentucky and submit its quiz. 
    • Not required of volunteers under the age of 18
    • Required every THREE years for those over the age of 18
  • Complete the online Volunteer Orientation Training and submit its quiz if you have not already done so.
    • Not required of volunteers under the age of 14.
    • ONLY required once during continued involvement with Special Olympics Kentucky.
    • Step 4 is only required if it has NOT already been completed. 

Q and A for SUBMITTING YOUR CLASS “A” VOLUNTEER APPLICATION
Question:  How do I submit my application to the state office for approval?
Answer: You are able to submit your application in the following three ways.  These three ways are for submitting the application only not the required Photo, Government Issued ID or Driver’s License.

    • By scanning and emailing to jharville@soky.org (preferred method)
      • Check out your smart phone APP store for apps that provide abilities to scan and email right from your phone.  
    • Faxing to the state office 502-695-0496
    • Postal mail

Question: How do I submit my required Photo, Government Issued ID or Driver’s License?
Answer: You are only able to submit the required Photo, Government Issued ID or Driver’s License the following two ways

    • By scanning and emailing to jharville@soky.org (preferred method)
      • Again check out your smart phone APP store for apps that provide abilities to scan and email right from your phone.  
    • Postal Mail
